Airbox lid not seated correctly
The airbox lid closed without seating, leaving a gap that admits unfiltered and unmetered air. It is both problems at once, which is why it matters more than it sounds: dust reaches the airflow sensor, and air enters without being measured.
Where the gap sits relative to the sensor decides the symptom. Upstream, it is mainly a contamination problem; downstream, it is a lean condition the module cannot correct. The complaint is often a whistle or intake roar that the owner mentions before any warning light. Check every clip is engaged and the lid sits flush — a lid held by clips at one end and proud at the other is the classic, and it is nearly always a service that was rushed.
