Disconnected or split sensor line
The small line connecting a pressure sensor to what it measures, off its stub or split. The sensor then measures atmosphere, which is a valid reading of the wrong thing — so the value stays in range and the module has no reason to doubt it.
A disconnected line reads flat and plausible, which is the dangerous outcome. Compare the reading against a known condition — an exhaust pressure that does not rise with revs, or a manifold reading that does not fall at idle, is a line fault rather than a sensor fault. They are disturbed for every job in the area and their stubs harden with heat, so recent work nearby is the strongest predictor.
