Toyota's 4.5 V8 diesel — in the Land Cruiser J200 and the Lexus LX.
It exists in two very different forms and the difference is the whole diagnosis. In markets with clean fuel it is a twin-turbo common-rail engine with a particulate filter; in markets without, it is a single-turbo engine with no filter at all. Advice written for one is frequently wrong for the other, and the vehicle's market of origin decides which you are standing in front of.
⚑ On the filtered cars the recurring failure is the ordinary one and the vehicle makes it worse. A Land Cruiser used as a town car never lets the filter reach regeneration temperature; the fuel injected trying washes into the sump, and a rising oil level is the warning. These vehicles are heavy enough that owners rarely notice the performance loss until a light comes on.
Injector wear is the second item, and it sets cylinder-specific codes worth trusting. Injector return-volume testing answers it cheaply and without diagnostic equipment.
Fuel quality is the variable that decides an engine's life here more than mileage does, and the filter and water trap intervals should be read as protection for the injectors rather than as servicing.
