The calibration string for an injector was typed into the module wrongly, or entered against the wrong cylinder.

Every modern diesel injector is flow-tested when it is made and carries its own correction code, because at pilot-injection quantities of a few cubic millimetres the manufacturing differences between two nominally identical injectors are large enough to matter. That code has to be entered per cylinder with a scan tool.

Two mistakes account for nearly all of these, and they behave differently.

A transposition — two codes swapped between cylinders when a set was fitted — leaves both cylinders corrected by the other's data. It usually sets a code, and finding one is a reason to verify the whole set rather than just the one named, because a transposition is by definition a pair.

A mistyped character is the more dangerous case, because it may set no code at all. These strings are long, printed small, on a part covered in diesel and often already fitted where the label is awkward to read. If the mistyped value is still structurally valid, the module accepts it and applies it for ever — one cylinder quietly fuelled wrong, with nothing on the dashboard to say so.

The check that settles it is reading the stored codes back and comparing them against the labels on the injectors themselves. It takes a few minutes, it is the only way to catch the silent version, and it is worth insisting on after any injector work.
