Damaged or bent throttle plate
The throttle plate itself distorted, so it no longer seats squarely in its bore. The plate seals the bore at idle, and even slight distortion leaves a gap that admits air the module did not command.
The symptom is a high idle the module cannot pull down, together with adaptation values driven to their limit, because the throttle is effectively open when commanded shut. The usual cause is impact — something drawn through the intake, or damage while the throttle body was off — so recent work or a failed air filter upstream is the strongest predictor. Look into the bore with the duct removed: a plate that does not sit flush, or shows a witness mark on one edge, is visible immediately and is not something cleaning will fix.
