Cause
Faulty oxygen sensor
The sensor that tells the engine module whether the mixture is right. A genuine wear item, poisoned slowly by combustion products, and it gets lazy rather than failing outright.
A slow sensor still produces a plausible-looking signal, which is why response time matters more than voltage when judging one — and why a sensor that looks correct at idle can still be the fault under load.
