P0AFE: Hybrid/EV Battery System Voltage Too Low for Voltage Step Up Conversion

This code describes a consequence — the battery too low for the drive electronics to work from. The cause codes stored alongside (state of charge, system voltage low) are where the diagnosis lives; the P0A7D storage and run-dry questions apply.
What this code means
The pack's voltage has fallen below the minimum from which the boost converter can step it up — the drive system's input floor, crossed.
Many hybrids do not feed the motors pack voltage directly: a boost stage steps a couple of hundred volts up to five or six hundred for the inverter, and that stage has a minimum input below which it cannot regulate. This code is that minimum violated — the consequence stamp on a pack that P0AFA or P0A7D describes the cause of. Read it as the system explaining exactly why drive is unavailable: not a boost converter fault, but a floor with nothing under it.
