Cold start EGR is a newer idea than EGR itself, and it runs against everything older practice assumed.
Classic EGR is disabled when the engine is cold. Recirculating exhaust into a cold engine hurts combustion stability, so for decades the rule was simple: no EGR until warm.
Tightening NOx limits changed that. The minutes after a cold start are when a catalyst is not yet working, so untreated NOx goes straight out — and EGR is the one measure that reduces NOx before the catalyst rather than after it. So modern engines admit a carefully metered amount of exhaust while still cold, accepting slightly rougher running to cut what leaves the tailpipe.
Which makes this family's symptom distinctive and easily dismissed: it is a fault that only exists in the first few minutes. Too much flow and the engine runs rough, stumbles or stalls when cold and is perfect once warm. Too little and nothing is felt at all — only the emissions test knows, which is why the insufficient-flow variant is a code with no symptom and a real consequence.
The cause is almost always soot. EGR passages carry hot exhaust into a cold intake, which is precisely the condition that deposits carbon, and a passage narrowed by years of it restricts flow long before anything fails electrically.
Excessive is the variant the driver meets. Too much exhaust in a cold engine produces rough running, stumbling, or a stall on the first pull-away — and it clears completely once warm, which is exactly why it gets reported as intermittent and dismissed.
