The driver genuinely pressing accelerator and brake at once, which the module records. It is a real event rather than a fault, and modern vehicles respond by giving the brake priority and cutting engine torque.

It is worth naming because the stored code reads like a sensor conflict. Left-foot braking, holding a vehicle on a hill, or a manoeuvre in a tight space all produce it legitimately. Check the freeze frame: both pedals applied at low speed for a short period is a driving event, and the same code appearing with the accelerator at rest is not. The other route is a stuck pedal or a brake switch reporting permanently applied — a failed brake pedal stop pad does exactly that, and then the vehicle cuts torque whenever the accelerator is used.

Typically an easy fix. Often worth checking before paying anyone to look.
