Cracked or perished EVAP hose
A vapour hose gone hard with age and heat, so the sealed fuel system no longer holds pressure. The system is only tested occasionally and under specific conditions, so the code appears long after the crack did — and the split is often only open when cold, which is why a leak test on a warm car can pass.
