Faulty EGR position sensor
The sensor reporting how far the exhaust gas recirculation valve has actually opened. The valve works in soot, so a position that disagrees with the command is far more often a valve stuck part-way than a lying sensor — and the sensor is frequently built into the valve, which makes the distinction academic when it comes to the part.
Typically an easy fix. Often worth checking before paying anyone to look.
