Faulty fuel pressure regulator
The electrically controlled regulator itself at fault — its winding, its internal valve or the part as an assembly — as distinct from one that is simply jammed. The separation matters because the two produce different evidence: a stuck regulator leaves a clean control circuit and a pressure that will not follow the command, while a failed one usually reports an electrical fault in the drive circuit as well.
On engines carrying more than one regulator the designator is doing real work. Codes naming regulator B are about the second device in the system — commonly the volume control on the pump's inlet side where regulator A sits on the rail — so a page naming B is not describing the part most people picture when they hear "fuel pressure regulator". Establish which physical device the code names before ordering anything, because the two sit at opposite ends of the high pressure circuit.
A regulator is also blamed for faults it merely reports. It can only hold the pressure the pump is able to supply, so a worn pump, a restricted filter or air drawn in on the suction side all present as a regulator that is not achieving its target. Compare commanded against actual pressure with the engine under load — a regulator at its maximum duty and still short of target is being asked for fuel that is not arriving.
