Cause
Wiring shorted to ground
A signal or supply wire touching the body or another earthed surface, so the voltage the module expects is pulled toward zero. The module normally reports this as an out-of-range low reading rather than as a short, which is why the code often names a sensor that tests perfectly well on the bench.
Insulation wears through where the harness crosses an edge, a bracket or a heat shield — so the damage is at the point of contact rather than at either end.
