Cause
Contaminated oxygen sensor element
The sensing element poisoned by something that should not be in the exhaust — coolant from a head gasket, oil past worn guides, silicone from the wrong sealant, or leaded fuel. The sensor is the victim rather than the fault, so a replacement fitted without finding what poisoned it is consumed the same way.
A new sensor that fails again quickly is the strongest evidence there is that something upstream is burning what it should not.
