What to do
Be honest about the journeys: mostly short trips is the usual whole story, and one long motorway run under load is both the test and often the cure. Check the oil level — above full means fuel dilution from interrupted burns, and that wants an oil change now, not at the next service.
What this code means
Regenerations are running too long — starting, burning, and failing to finish inside their allotted window.
The classic cause is the driving, not the car. A regeneration needs sustained heat, which needs sustained driving; a journey that ends mid-burn aborts it, the soot remains, and the module tries again next trip. Short-trip patterns produce exactly this code — regenerations forever starting and never completing — and each attempt has a hidden cost the corpus's diesel pages keep circling: the extra fuel injected to heat the exhaust partly washes past the pistons into the sump. Check the dipstick for a level that has risen — oil above full, smelling of diesel, is interrupted-regeneration history written where anyone can read it, and oil dilution is its own engine-wear problem.
The mechanical causes — a fouled filter that resists burning, a lazy temperature sensor undershooting the target — are real but second: the usage question is free and answers most of these.
