Faulty exhaust gas temperature sensor
The sensor protecting the particulate filter and turbocharger by watching exhaust temperature. When it reads wrongly the module refuses or aborts regeneration rather than risking a fire, so the filter fills, and the filter gets replaced while the sensor that stopped it clearing is still fitted.
