Seized manifold runner spindle
The shaft the intake flaps pivot on, bound with carbon. The actuator is what reports the fault and the spindle is what caused it, so an actuator fitted to a seized spindle fails again — and on many manifolds the spindle is not serviceable, which turns a cheap-looking code into a manifold.
