Wiring shorted to voltage
A signal wire touching a supply, so the module reads a high value the sensor never sent. It frequently damages the module input as well as setting the code, which is why the wiring has to be found and repaired before a replacement part goes in — a new sensor wired into an unrepaired short fails the same way.
