The module learns the clutch the way your leg learns one. No two clutches bite at the same point or carry the same torque per millimetre of travel, and both drift as the material wears. So the module continuously measures where its clutch bites and how much it transmits, and updates a stored model — that adaptation is why a gearbox still shifts smoothly at 150,000 miles on a clutch half worn away.

At limit means the learning has run out of room. Adaptation absorbs change, but only within a window, and the stored values have reached its edge. The software can no longer compensate for what the hardware has become. A clutch worn to the end of its service life is the ordinary reason, and this code is often the first formal notice — arriving before slip, judder or smell make it obvious.

The other route here is a lost baseline rather than a worn part. Replace a clutch or an actuator and the learned values now describe a part that is no longer in the car; interrupt the wrong procedure and they are simply gone. A scan-tool teach-in re-baselines the system — so a gearbox that shifts badly immediately after clutch or actuator work needs calibration, not a second clutch.

This code straight after a repair is good news, relatively speaking: the fix is a procedure, not parts. The same code on an original clutch at high mileage means the opposite — budget for the clutch, and treat smooth shifting in the meantime as borrowed time.
