Cause
Carbon deposits on intake manifold flaps
The swirl or runner flaps coked into place. They sit directly in the incoming air on direct-injection and diesel engines, where nothing washes them, so the deposits are structural rather than accidental — and replacing the actuator without freeing the flaps kills the new one the same way.
