P016C: Closed Loop Air/Fuel Ratio Control At Limit - System Too Lean

What to do

Say whether the problem is worse at idle or under acceleration — it is the single most useful thing you can report here. Worse at idle points at an air leak; worse under load points at fuel supply, and those are completely different repairs.

Mention any hissing or whistling from the engine bay, especially at idle, which suggests an air leak.

Say when the fuel filter was last changed, since a restricted filter is a common and inexpensive cause on the fuel side.

Do not leave it running lean for long. A lean mixture burns hotter and can damage the catalytic converter and, in the worst case, the engine itself.

What this code means

The module has added as much fuel as its closed loop authority permits and the mixture is still lean.

This code is the sound of a correction running out of room, which makes it more informative than an ordinary lean code. The module is not reporting that the mixture drifted — it is reporting that it tried to fix it, went to the end of its adjustment, and failed. The fault is therefore larger than trim can absorb.

Two families of cause, and separating them is the whole diagnosis.

Unmetered air entering after the airflow sensor. The module fuels for the air it measured; anything arriving later is unaccounted for and leans the mixture out. Split intake hoses, a perished PCV hose, a leaking intake manifold gasket, a loose dipstick tube or a failed brake servo hose all do it. A leak of this kind is usually worse at idle and improves under load, because manifold depression is highest at idle.

Fuel that is not arriving. A weak pump, a restricted filter, a failing pressure regulator or blocked injectors. This pattern is the reverse — worse under load and better at idle — because demand exposes a supply shortfall that light throttle does not.

That single question, 'is it worse at idle or under load?', separates the two halves before anything is dismantled. It is the most useful thing to establish here.

And check the measurement itself: a mass airflow sensor reading high makes the module fuel for air that was never there.
