Heat damage to sensor or wiring
Insulation baked hard and cracked where a lead runs close to the exhaust. The lead usually fails before the sensor does, so a sensor condemned on a reading taken back at the module may measure perfectly at its own connector — and the fault often appears only when hot, clearing as the vehicle cools.
