Worn high pressure fuel pump
The pump generating injection pressure on a direct-injection or common-rail engine. On a diesel its only lubricant is the fuel, so water or contamination wears it — and the metallic debris travels downstream into the injectors, which is how a cheap filter job postponed becomes a whole fuel system.
Typically specialist work. Needs equipment or training most home workshops do not have.
