P2192: System Too Rich at Higher Load Bank 1

What to do

Expect poor economy, possibly black smoke under acceleration, and a flat or heavy feel. Do not drive hard with this unrepaired — excess fuel under sustained load overheats the catalytic converter and can destroy it, which turns a sensor-sized bill into a converter-sized one. Ask for the air filter and airflow sensor to be checked before injectors are considered.

What this code means

Bank 1 runs rich under load. Less common than the lean variant, because the module normally has authority to reduce fuel and will do so — so this code means the excess persisted despite correction.

The usual source is a measurement error that scales, since the fault appears where flow is greatest. An airflow sensor over-reporting produces fuel metered for air that is not there, and the error grows with the throttle. On a forced-induction engine, a boost pressure sensor reading high does the same thing through a different route.

A restricted air filter belongs here for a reason that is often overlooked: it does not merely reduce power. On a speed-density system the module infers airflow from manifold pressure and engine speed, and a filter restrictive enough to alter that relationship makes the inference wrong in the rich direction under load.

The consequence is the one named at P2178 and it is worse here, because load means volume: unburnt fuel arriving at a hot converter under sustained load is the fastest way to destroy one.
