What this usually means

A strong petrol smell at the tailpipe means fuel is passing through unburnt, and it loads the catalytic converter heavily. That is the reason to act rather than tolerate it: the converter is the expensive component and it is being destroyed by the fault rather than causing it.

The mechanism is the same one behind a flashing engine management lamp. Raw fuel reaching a hot catalyst ignites on the catalyst rather than in the cylinder, and the resulting temperature can melt the ceramic substrate — so a smell that seems merely unpleasant is a countdown on a costly part.

The population is short. A misfire putting one cylinder's charge straight out of the exhaust valve. A leaking injector adding fuel that never burns. A mixture running rich because the management is being told something wrong — a coolant temperature sensor reading cold, an air mass meter under-reading, or excessive fuel pressure. Or an oxygen sensor that has stopped correcting, so nothing closes the loop.

Read the fuel trims first, because they say directly whether the management knows it is rich. Trims heavily negative means the management is fighting an over-fuelling it can see; trims near zero with a rich smell means it does not know, which points at the sensor doing the measuring rather than at the fuelling.

A smell only when cold that clears as the engine warms is closer to normal, because a cold engine genuinely runs rich — but one that persists warm is a fault every time.
